Your shelter veterinarian is probably in the best position to let you know if surgery is absolutely necessary. It is just hard to tell without seeing the kitten. Many "club feet" are fairly minor and the kitten will do well without surgery while others may absolutely need to be operated on. Even if surgery is not necessary, this kitten will likely have more expenses down the road due to possible arthritis, etc. I would also make sure there are no other defects - often times if there is one congenital abnormality there is another.
