We can suspect worms on physical exam if we see a pot-bellied appearance or dull coat or based on symptoms like diarrhea. A fecal sample is the best way to detect worms but does not pick up all kinds of internal parasites nor does in catch them all the time. The fecal sample looks for microscopic eggs from the parasites and these eggs are not shed all the time. In some cases, it is just easier to give your cat a worming medication...just in case.
